Huron Perth And Area Ontario Health Team's Heart Failure Program Named Finalist in Ontario Health System Quality and Innovation Awards

The Huron Perth & Area Ontario Health Team's Heart Failure Program has been named one of three finalists in the Improved Population Health category of the 2024 Ontario Health System Quality and Innovation Awards.
The recognition highlights the program’s exceptional efforts in enhancing heart failure care for Huron and Perth and area residents. The Ontario Health System Quality and Innovation Awards celebrate the transformative work being done across the province to address evolving healthcare challenges such as chronic disease management, mental health, and social determinants of health. The Improved Population Health category, in particular, honours initiatives that demonstrate measurable success in improving health outcomes for specific communities.

The Improved Population Health Award highlights projects that create meaningful and lasting change within healthcare. It acknowledges the importance of integrated, team-based care that is focused on improving the health of entire populations. Criteria for the award included:

Addressing local population needs and improving social determinants of health.
Prioritizing disease prevention and screening efforts.
Demonstrating clear, positive health outcomes.
Although the Heart Failure Program did not win the top prize, becoming a finalist is a remarkable achievement and a testament to the program’s success in enhancing the quality of life for heart failure patients. The nomination highlights the innovative strategies implemented by the team and their impact on population health across Huron Perth and Area.
